Primary care physician tool

Service facilitating the supervision and exchange of health information on the patient's care path

The Primary Care Physician Tool, operational since 2014, is a service made available to general practitioners or pediatricians providing primary care in non-hospital settings and who have signed, jointly with the patient, a "refering doctor declaration".

For a patient to be eligible for the medical consultant device:

  • he/she must have at least one long-duration-disease (LDD), the severity or chronic nature of which requires prolonged treatment and a significant coordination effort due to the intervention of several care providers. ALDs are defined in appendix 1 of the agreement between the "Caisse nationale de santé" and the "Association des médecins et médecins-dentistes" published in the Memorial of October 20, 2015. This is a list of 32 long-duration-diseases, such as multiple sclerosis, Alzheimer's and Parkinson's disease, cystic fibrosis etc. To consult the list click here.
  • he/she must have an activated electronic health record (DSP), the personal and secure electronic health record, made available by the Agence eSanté.

The Primary Care Physician tool, offered by the eSanté platform, allows medical consultants to supervise and exchange health information on the patient's care path, and thus facilitates them in fulfilling their missions specified in article 19bis. of the Social Security Code: coordinate care and ensure that medical monitoring is optimal.

More specifically, this coordinated follow-up is facilitated by means of two documents which, as part of the primary care physician tool, must be placed in the patient's DSP:

  • Summary of benefits : this statement is paid annually by the “Caisse nationale de santé” to the patient's DSP (the “Caisse nationale de santé” can only send documents to a patient's DSP, it cannot in any case read or modify the information contained in the DSP). The summary of benefits shows the benefits covered by sickness-maternity, accident and long-term care insurance for the past three years. It can be consulted by the patient and his medical consultant.
  • Patient summary: this is a standardized document containing the most important medical information to ensure safe and secure care. The medical consultant updates this document during each contact with the patient to include any new clinical or therapeutic element relevant in relation to the patient's state of health and useful for the coordination of care between healthcare professionals. At the patient's request, the medical consultant explains the content of the patient summary and gives him a free copy. This document can be consulted by the doctors taking care of the patient, provided that the latter has given them the right of access to his DSP. 

The medical consultant accesses the tool from the multiservice portal www.esante.lu, or via his business software, if compatible with the eSanté Platform. He accesses it through his eSanté Platform account, to which he connects with the personal connection information he has obtained from the Agence eSanté and by using his personal LuxTrust device.
