You can close your DSP yourself by logging in and clicking on the ‘Traçabilité’ tab and then on ‘Fermeture du dossier’.
If you cannot/do not wish to close your DSP yourself, you can request that it be closed using the ‘Request for closure’ form, which can be downloaded below.
However, we would like to draw your attention to the fact that closing your DSP may complicate the exchange and sharing of your health information between the various healthcare professionals treating you.
